Please be sure you have read all RULES & ELIGIBILITY REQUIREMENTS before submitting. By submitting your recipe, you agree that you have read and understood the requirements.
Submission form must be filled completely. Failure to do so will result in immediate disqualification.

NAME. Fill out complete legal first and last name

EMAIL. Must be a valid email address

CURRENT EMPLOYER. Must provide current and primary place of employment with a manager’s name for reference

NAME OF PUNCH. Must provide name for Punch recipe (don’t be vulgar)

CHICAGO CONNECTION. Please provide a brief (think Twitter limit on characters) of why you chose this drink and how it embodies Chicago to you

RECIPE. Must provide full details of ingredients (with measurements) and steps of service for preparation. Full description of garnish must be included. If including custom syrups or tinctures, full ingredients and preparation of said syrups/tinctures must be included as well.
